PARTICIPANT VOLUNTARY LABORATORY CONSENT, PHOTOGRAPHY, ACADEMIC HONESTY, COPYRIGHT PROTECTION, AND PRACTICE ACT LIMITATIONS AGREEMENT
I, the undersigned, do hereby acknowledge my voluntary participation in the course/seminar entitled ____________________________________ instructed by _______________________________________. I understand that the techniques taught in the course named above are advanced and may carry personal risk to me. I agree to report any pre-existing conditions, precautions or contra-indications to both the Instructor and any participant at the course who may be asked to practice the demonstrated techniques on me. Questions regarding pre-existing conditions should be directed to the instructor PRIOR to the first lab session.
DEMONSTRATION IS NOT TREATMENT. Techniques of assessment are part of the instructed laboratory sessions for the course listed above and are NOT to be construed as treatment. Demonstrating an examination or treatment technique on a patient volunteer or course participant is therefore not defined as treatment in this regard. MAPS faculty is not expected to treat participants during or outside of class hours.
VOLUNTARY LABORATORY CONSENT. During course hours for the seminar listed above, I agree to practice only the techniques taught at the course and to strictly follow the directions of the instructor regarding the application of those techniques. I understand that no course participant is compelled to perform any technique, or have any technique performed upon him or herself against his or her wishes. I understand that if I choose to participate in any laboratory sessions, I am responsible for any effect of my application of the demonstrated techniques on any patients and/or my fellow participants, including any effect on any patient/participant with or without any pre-existing condition/(s). Neither the Host/Sponsor facility, MAPS, nor the Instructor, can be held responsible or liable for any accident or injury arising out of the laboratory sessions for the course/seminar listed above, or at any other time during or subsequent to the course/seminar herein.
PHOTOGRAPHY. Recording, photography or videotaping is not permitted.
ACADEMIC HONESTY. All home study, in-class and outside class assignments and assessments, including those reported in the MAPS Home CEU study Survey, require participants to answer truthfully and to participate both honestly and with the utmost in integrity throughout the course, reflective of the standards required by medical professionals and the APTA. By signing this waiver, the participant agrees that any completed assignments and/or assessments were indeed completed by the course participant and not by another individual or individuals without any unauthorized assistance. Violation/(s) of this policy will prevent a participant from successfully completing the course listed herein.
COPYRIGHT PROTECTION. ALL MAPS course materials, including but not limited to, course manuals, pre-reading, examinations and overnight sheets, that may be distributed prior to, during or after the seminar are protected by US Federal Copyright Law and are registered with the US Copyright office. ALL cases of copyright infringement will be aggressively pursued by MAPS to the fullest extent of the law. Federal Copyright law provides for severe civil penalties, including statutory damages of up to $300,000 and criminal penalties of up to 10 years imprisonment. Photocopying, reproduction or dissemination of MAPS materials by any means is strictly forbidden. Recording, photography or videotaping is prohibited.
PRACTICE ACT ISSUES. In accordance with APTA HOD RC 31-01, I understand that the material taught in this course is not intended to be used outside the scope of any participant’s personal abilities, licensure or relevant regulations. Furthermore, elements of patient/client care as instructed herein are considered Physical Therapy only when performed by or under the direction of a Physical Therapist in accordance with APTA policies, positions and standards.
